It may look cute but there are three concerns that I have with it. One is if it is for a child, they could get the strap caught around their necks, the second concern is it would put a real strain on the place where the button is attached that would cause it to tear through the quilt and you really don't want to use buttons with small children that could come loose with wear and be swallowed. If you want a strap of some kind, I would suggest making two short straps attached at the end of the quilt overlapping using velcro.
